Introducing the Tweed Urgent Care Service and how it can alleviate the challenges facing GPs and aged care staff in providing urgent care for older people at home.

Healthy North Coast will facilitate lively discussion on integrating support for older people in the Tweed, improving outcomes for all.

Be introduced to:

  • The UCS team
  • The services on offer and referral pathways
  • Embedding common practices– MyHealth Record, ACD’s, triaging and clinical handover

Topics include:

  • Insights from recent sector – wide consultation
  • Strengthening MyMedicare -Primary Care for aged care residents
  • Enablers to streamlining care including tele-health, after hours support, relationships and capability

Who should attend:

  • GPs and other primary care professionals with older patients
  • Clinicians and senior team members of aged care providers (residential and community)
